PRF under-eye treatment uses platelet-rich fibrin from your own blood to improve thin, crepey skin and soften mild hollowing beneath the eyes. What PRF Under-Eye Injections Do
Platelet-rich fibrin, or PRF, is prepared by drawing a small amount of your blood and spinning it at a slower speed than standard platelet-rich plasma. That gentler process keeps platelets and white blood cells within a fibrin network, which releases growth factors gradually rather than all at once. Injected beneath the thin skin of the lower eyelid, PRF supports the skin’s own repair response: new collagen, improved thickness and better hydration over time.
Early laboratory work helps explain the effect. Sclafani and McCormick (Archives of Facial Plastic Surgery, 2012) took biopsies after injecting platelet-rich fibrin matrix into the skin of volunteers and found activated fibroblasts and new collagen as early as seven days, with new blood vessel formation by about day 19. More recent clinical research is encouraging but measured. In a 2025 systematic review of 14 studies, Sollitto and colleagues (Journal of Cosmetic Dermatology) found that PRF was associated with improvements in under-eye texture, wrinkles and crepiness, while PRP showed stronger evidence for pigmentation. Neither was clearly superior, and the authors called for larger trials.
In practical terms, PRF under-eye injections are best for crepey or thin skin, fine lines, mild tear-trough hollowing and a tired look caused by skin quality. They are not a substitute for structural volume, and they do not remove bags.
How PRF EZ Gel dsds From Standard PRF
Liquid PRF spreads easily and is absorbed fairly quickly. With EZ Gel, part of your plasma is gently heated to form a smooth gel, which is then blended with liquid PRF just before injection. The result is a soft, injectable material with more body than liquid PRF, which allows it to provide subtle support in a hollow while delivering growth factors to the surrounding skin. Rahbari and colleagues (Facial Plastic Surgery, 2026) describe PRF gel as an autologous alternative to hyaluronic acid filler for periocular rejuvenation, suited to patients seeking a conservative approach, including those who have had problems with hyaluronic acid under the eyes, and they report progressive volume improvement with repeated treatments.

PRF EZ Gel
PRF EZ Gel is our choice for patients with crepey skin, fine lines and mild-to-moderate hollowing who prefer a natural, gradual result. Improvement builds over weeks as collagen forms, and best results are typically seen after two to three treatments. It suits patients who are cautious about filler, prone to puffiness with hyaluronic acid, or want to improve skin quality and hollowing at the same time.
Eye Boost With Hyaluronic Acid Filler
The Eye Boost uses hyaluronic acid filler to fill under-eye hollows and create a smoother transition between the tear trough and cheek. It gives a more immediate and predictable volume correction than PRF and is often the better tool for a deeper, well-defined hollow. Hyaluronic acid can also attract water, so it is not ideal for every patient, particularly those who already retain fluid under the eyes. When hollowing extends into the upper cheek, an Ultimate Eye Boost addresses the mid-face so the under-eye area is supported from below.
PDGF
Our Ariessence Pure PDGF+ treatment is a purified concentrate of platelet-derived growth factors without platelets or fibrin. It can be injected or applied after microneedling and is a strong option for crepey, thinning under-eye skin and uneven texture. Because it is a liquid rather than a gel, PDGF focuses on skin quality rather than filling a hollow.
Some patients benefit from a staged combination, such as an Eye Boost for structure followed by PRF or PDGF for skin quality, with healing time between steps.
Honest Limits: When PRF Is Not the Answer
Not every under-eye concern is a PRF concern. Puffy under-eye bags are usually caused by lower eyelid fat pads that have pushed forward, sometimes combined with fluid retention. PRF does not shrink or reposition fat, and adding volume around a true bag can make it look larger. A 2026 multispecialty systematic review by Beer and colleagues (Dermatologic Surgery) concluded that surgery is preferred for herniated fat and poor lower eyelid support, while fillers remain foundational for volume loss and microneedling targets texture and fine lines. PRF also has a modest effect on dark circles caused by pigment or visible blood vessels, and it does not tighten loose eyelid skin the way an energy-based treatment can. For laxity and crepiness of the eyelids themselves, our Sylfirm X non-surgical eye lift may be a better match.
How Many PRF Under-Eye Treatments Are Needed?
Most patients need two to three PRF EZ Gel treatments under the eyes to see their best results. Patients with deeper hollowing or more advanced skin thinning sometimes benefit from an additional session. We space treatments several weeks apart so collagen has time to develop, and we review your progress photos before each appointment to decide whether another session will add meaningful improvement.
Longevity varies. Many patients enjoy their results for several months to about a year, depending on age, skin quality, lifestyle and the number of sessions completed. The Sollitto review noted that PRF improvements in some studies softened by around six months, which is why we discuss maintenance from the start rather than presenting PRF as a one-time fix.
Ultrasound for Tear-Trough Safety
The tear trough is one of the most delicate areas of the face. Skin is thin, important blood vessels run nearby, and many patients have filler placed years ago that is still present. At Beauty Boost, Anisa uses ultrasound imaging for vascular mapping, locating previously placed filler and assessing tissue planes, and our ultrasound-guided treatments bring that same visibility to the under-eye area.
This matters for PRF in two ways. First, ultrasound helps our injectors see vessels and tissue depth before and during placement, supporting precise, safer injection. Second, it reveals old hyaluronic acid filler that may be causing puffiness or a bluish tint. When that is the case, we may recommend dissolving it with a Reset Boost before starting PRF, so we are treating your natural tissue rather than layering over old product. Because PRF EZ Gel cannot be dissolved the way hyaluronic acid can, careful placement from the start is essential.
What to Expect
Your appointment begins with a consultation and assessment of your under-eye area, cheeks and skin quality, plus a review of your medical history and any prior filler. We draw a small amount of blood and prepare your PRF EZ Gel while a topical numbing cream takes effect.
Afterward, mild swelling, redness and tenderness are common for a few days, and bruising is possible. Initial swelling can make the area look fuller at first, so the true result emerges gradually over the following weeks. PRF may not be appropriate if you are pregnant, have a platelet or bleeding disorder, take anticoagulants or have an active infection near the eyes. Do not stop a prescribed blood thinner without direction from the clinician who prescribed it.
Reading PRF Under-Eye Before and After Photos
When reviewing PRF under-eye before and after images, look for the same lighting, angle and expression in each photo, since overhead light deepens hollows and soft frontal light hides them.
